The service provides medicine information, enables patients to understand the importance of their medicines, and to improve communications between the hospital and GPs within the Preston area.

Improving medicines undertaking and taking, making improvements, resolving medicines related problems and improving communication of information are all issues addressed by this service. Results show reduced re-admissions to hospital.

How does the Service Work?

Patients (with consent) are referred to us by other healthcare professionals based on the following criteria:

Long term condition
Aged 60+
Recent hospital admission
4 or more medicines
Preston resident

The technician will then carry out a medicine check either whilst in hospital or in the patients own home following discharge. The technician, where necessary, will help to make improvements and/or changes to ensure that the patient/carer is able and confident with managing their own medicines.

We will answer any questions you or your family/carers may have regarding your treatment liaising with other services where necessary.

How will this help me?

During the visit the technician will ask questions about your medicines and how you order your repeat prescriptions from your GP. Any concerns you have can be discussed. They may include forgetting to take your medicines, any changes that have been made and how to store your medicine safely.

We can explore with you ways to help, eg, colour coding.

Any medicines identified as no longer required or out of date can be removed for safe disposal with your consent.

What happens following the review?

The technician will discuss any further action and liaise with the GP, community pharmacy or others involved in your care before confirming the arrangements with you.

No clinical changes will be made to any medicines without the consent of your GP.

Ongoing care

The technician may arrange follow up visits to ensure the patient is continuing to get the most benefit from their medicines.
