What to Expect Following Discharge From Our Care.

You have been treated in the Emergency Department. You have been thoroughly examined and are now considered fit to be discharged. The following will instruct you what to do when you leave.

When you leave our care in the Emergency Department, we want you to be confident and clear about:

What has happened to you today
What may happen next
Where you can get further information if you need it
What actions you may need to take for follow-up appointments, if necessary.

These are our five commitments to you when you are discharged from our care:

You are clear about what has happened today and what tests have been completed whilst you have been in the Emergency department. If you have any questions, please ask before you leave the Emergency department. A discharge letter which explains what has happened today will automatically be sent to your GP electronically. You may be given a personal copy of this.
You have received any relevant information about what to do next. For example, how or where to get medications from the Boots hospital pharmacy, or where to book follow up appointments that you may need.
You have been made aware of any possible ‘worrying signs or symptoms’ to look out for when you have been discharged, specifically relevant to the reason you attended today.
You know how to take any medication you may have been supplied with or prescribed in the Emergency department and you are clear about where to find the information about any possible side effects these may have. Our patient medication helpline is available for you if you any questions about your medicines after you have left hospital. The Freephone number is 0808 2810 188 and the line is manned from 2.30–4.30 pm each day, Monday to Friday. Alternatively, the hospital Boot’s pharmacy number is 01737 766 365 (8–8.30 pm Mon–Fri, 10–4 pm Sat–Sun)
You know who to contact if you are worried about your condition. Please contact your GP or you can return to the Emergency department if you are not getting better or contact other specialists as advised.

Please speak to a member of staff if you need any clarification in regard to these commitments.

It is always important to know and recognise the signs and symptoms of sepsis.

Sepsis is the body’s reaction to an infection (previously known as septicaemia or blood poisoning). Please do seek medical advice through your GP, NHS 111 or 999 if you do develop any of the following symptoms:

Feeling very hot and flushed
high temperature
aching muscles
feeling abnormally very tired
prolonged sickness/diarrhoea
not feeling like eating
feeling confused or having slurred speech
