The MySelfCare system has been designed to make it easier for you to present aftercare/discharge advice and recommendations for your service users.
Our starting point has been the Patient Information cards BOE provides to your department/Trust. However with this digital platform we can offer so much more, including call to action tools.
We can add Call 111 links that will allow a mobile phone to call the number at a click. We could include department or specialist numbers as well. We can also link to emails as well as websites.
Another great feature is embedding videos, very useful if you want to advise specific exercises.

or an across body stretch for a shoulder condition
Trust Messaging
MySelfCare offers the option to add specific Trust messages either within the patient information, or as a dedicated related post.
Anonymised Analytics
Our platform allows us to offer feedback on service users experience with the Trust. We can also share general data in relation to the number of service user visits to the hospital that have used the system, as well as which conditions they viewed.
Integration
Apart from feedback forms, we can also produce documents that allow/require digital signatures (such as for consent forms) that can be completed online and emailed to both the service user and the Trust, which can help reduce paper work.
Our system can also link to your online appointments calendars – allowing service users easier access to these facilities.
No Limits
There are really no limits to the digital scope of the system, and we are confident it compliments the great service offered by our NHS.
